Parent Involvement Survey for Parents of Children Receiving Special Education Services

All parents of school-aged children and youth who receive special education services are encouraged to complete the Virginia Department of Education's annual Parent Involvement Survey. This is an opportunity for parents to provide input to help guide efforts to improve services and outcomes for their children. Your responses will be kept anonymous and never be personally linked to your child. The survey is available in English and Spanish, and will be available from September 15, 2012 through June 30, 2013. This online survey may be found at:

If you are unable to take the survey online, please contact Tracy Lewis, Family Resource Coordinator, at 524-3405, ext. 105 or, for a paper survey. All paper surveys include a pre-addressed, postage-paid return envelope, and responses will be sent directly to the Virginia Department of Education.

If you have questions related to the Parent Involvement Survey, please contact Patricia Haymes at the Virginia Department of Education.  She can be reached via email at or by phone at (804) 786-0116.
